Home Instead provides personalized, non-medical in-home care that helps older adults remain safe, comfortable, and independent in their own homes. Our Caregivers assist with companionship, personal care, meal preparation, transportation, medication reminders, and other daily activities.
Home Instead is seeking an experienced, relationship-driven Client Care Coordinator to serve families throughout the Charleston area. This highly client-facing, field-based position is ideal for someone who can build trust quickly, help families navigate important care decisions, and maintain strong client relationships throughout their time with Home Instead.
This is not primarily a desk-based care coordination position. A significant portion of the role is spent conducting initial in-home care assessments, Quality Assurance visits, and client follow-up throughout Mount Pleasant, downtown Charleston, West Ashley, James Island, Kiawah Island, Seabrook Island, and surrounding communities.
A company vehicle is provided for business-related travel.
What You’ll Do
- Conduct in-home care assessments with prospective clients and their families to understand their needs and recommend appropriate services.
- Build trust with families and confidently guide qualified prospective clients through the decision to begin services.
- Maintain a high level of conversion from qualified care consultations to new clients.
- Complete service agreements, care plans, required documentation, and timely follow-up for new clients.
- Coordinate with Scheduling and Client Care to support timely, successful starts of care.
- Conduct ongoing Quality Assurance visits with existing clients and families.
- Identify changes in client needs and recommend adjustments to care when appropriate.
- Recognize opportunities where additional care may improve a client’s safety, well-being, or quality of life.
- Proactively address client concerns and work with the internal team to resolve issues and strengthen client satisfaction and retention.
- Maintain accurate and timely documentation in company systems.
- Manage a field schedule independently while maintaining consistent communication with the office team.
- Participate in the shared weekend on-call rotation, generally serving as backup approximately every fifth weekend for the existing on-call team.
- Represent Home Instead professionally throughout the Charleston community.
Who We’re Looking For
We are looking for someone who combines exceptional relationship-building skills with the confidence to ask for the business.
The ideal candidate has experience in home care, senior living, home health, hospice, healthcare admissions, healthcare business development, or another consultative, client-facing environment.
You may be a great fit if you:
- Have three or more years of client-facing, relationship management, admissions, business development, or consultative sales experience.
- Have demonstrated success converting qualified inquiries, consultations, or admissions into clients or customers.
- Are comfortable discussing pricing and helping families make decisions.
- Connect naturally with older adults and adult children, including families navigating stressful or emotional situations.
- Listen carefully and uncover what a family truly needs instead of simply presenting services.
- Are comfortable working independently and spending a significant amount of time in the field.
- Demonstrate sound judgment and strong problem-solving skills.
- Take ownership of client relationships and follow through on commitments.
- Are organized and capable of managing documentation and multiple client priorities.
- Communicate effectively with clients, families, Caregivers, and internal team members.
- Are willing to participate in a shared weekend backup on-call rotation.
- Have a valid driver’s license, an acceptable driving record, and the ability to travel throughout the Charleston service area.
Home care or senior care experience is preferred but not required. We are also interested in candidates with a strong track record in senior living sales and admissions, home health or hospice liaison roles, healthcare business development, or other relationship-driven industries.
What Success Looks Like
Success in this position means taking ownership of the client experience from the initial care consultation through the successful start and ongoing delivery of services.
Key measures of success include:
- Strong conversion of qualified care consultations into new clients, with a target of 80% or greater.
- Successful and timely starts of care.
- Strong 30- and 60-day client retention.
- Consistent completion of Quality Assurance visits and required follow-up.
- Proactive resolution of client and family concerns.
- Identification of changing client needs and appropriate opportunities to expand care.
- Accurate and timely documentation and communication.
- Effective collaboration with Scheduling, Client Care, and other office team members.
- Contribution to continued growth throughout the Charleston service area.
Compensation and Benefits
- Salary: $60,000–$65,000 annually, based on experience and demonstrated qualifications.

Schedule, Travel, and On-Call Rotation
This is a full-time, field-based position requiring regular travel throughout the Charleston service area, including Mount Pleasant, downtown Charleston, West Ashley, James Island, Kiawah Island, Seabrook Island, and surrounding communities.
Some scheduling flexibility is required because care consultations and client needs do not always occur during traditional office hours.
This position participates in the company’s shared weekend on-call rotation, generally serving as backup approximately every fifth weekend for the established on-call team. This position is not responsible for serving as the company’s primary on-call manager.
